This study aims to examine the effect of pesticide exposure risk on the incidence of respiratory diseases in farmers in industrial areas, with the Sustainable Healthcare Systems approach as a mediating variable. This study uses a quantitative approach, where data is collected through a survey involving 487 active pesticide user farmers in industrial and agricultural areas. The data collection instrument uses a questionnaire with a Likert scale, which is then processed using the Partial Least Squares - Structural Equation Modeling (PLS-SEM) method with SmartPLS software to test the relationship between variables. The results of the study indicate that the risk of pesticide exposure has a significant effect on the incidence of respiratory diseases in farmers, with the Sustainable Healthcare Systems approach acting as a mediator that strengthens this relationship. These findings confirm the importance of integrating environmental and health policies to mitigate risks faced by farmers in industrial areas. This study fills a gap in previous research by integrating the Sustainable Healthcare Systems approach in the context of pesticide exposure and its impact on health, especially on vulnerable groups such as farmers. This study provides practical implications for health and environmental policies, which can be used to formulate sustainable health system-based mitigation strategies, to protect farmers from the impacts of pesticide exposure and improve their quality of life.
